Change Life is a reality TV program on charity cosmetic surgery conducted by Vietnam Television in collaboration with the hospitals. After four seasons, the program has received positive feedback from the audience.
This year, Change Life is back in its 5th season. The notice on the recruitment has been published on the official fanpage of the program. The show is scheduled to air in October 2020.
Following are the recruitment information:
1. Contestants:
- Male and female citizens; aged 19 - 45, living in Vietnam.
2. The application dossier includes:
- Curriculum vitae certified by local authorities.
- Personal information: height, weight.
Headshot photo, size: 9cm x 12cm.
(Profile shot of the left side, the right side, top down shot and vice -versa, photo of the entire face)
- Full body photo (Front, back and left, right profile shots)
(*) a video clip made by candidates to introduce about themselves (duration of 45-60s) is an advantage for the selection round.
- The application must state how the facial/appearance malformation affects one’s life and one's wish after surgery. It is accompanied by a signed consent paper by the parents allowing their child to participate in the program.
- A photo of a valid passport in accordance with the law (if any).
Invalid application are those missing any of the documents listed above.
3. Application deadline: From now till the end of March 20, 2020
4. How to send documents (only select one way to send):
- Send the profile file as an image via Facebook Messeger, account Change Life VTV2.
- Send the application dossier directly or by post to the address: Department of Science and Education Programs, Vietnam Television, No. 43, Nguyen Chi Thanh, Ba Dinh, Hanoi. The application must specify as being sent to Change Life program.
